A worker at thermoforming company Duo-Form Plastics has died as a result of injuries suffered July 27 at the company’s Edwardsburg, Mich. factory.
Duo-Form executives declined to comment. The Michigan Occupational Safety and Health Administration said it was investigating and could not comment. It also said in an email that Duo-Form has no history of incidents with MIOSHA.
A local media report said police were called shortly after 3 p.m. after three-year employee David Kantz, 32, of Cassopolis, died after being injured in a press.
Duo-Form is a heavy-gauge thermoformer, established in 1975. It runs more than 15 thermoforming lines in its 190,000-square-foot facility, according to its website, and makes consumer and industrial products, including bath enclosures. Plastics News estimates its annual sales at $ 23 million.
